The main difference is where your Spirit, your Consciousness, is and what vehicle it is using to get there. There is more than one form of Journeying. Bardon discusses the two most important ones for most practitioners, those being what he calls Mental Wandering and Astral Projection. The difference between them is the vehicle you use to Journey. Mental Wandering utilizes the Mental Body, whereas with Astral Projection you separate the Astral Body (soul) from the physical body and use that as your vehicle.
There is also, in my own practice, Internal Journeying.
Rowe touches on this a bit with his Sacred Space practice, and I also use it for Lucid Dreaming and similar internal exercises.

The difference between imagination and Journeying is that with Journeying, you are actually...Journeying somewhere. With Mental and Soul Journeys, this is easily verifiable. You can just walk around your room, house or neighborhood (exactly as Bardon instructs) in order to look around and stuff and learn things. Then when you're back in your body, you go check those things to see if you actually saw them or imagined them.
The Plastic Imagination is a powerful tool for concentration, but you need to learn the difference between imagination and real informational input. Not learning this difference quickly leads to delusion. This is why Bardon teaches you Imagination in step two and three and why he only gives instructions for things that can lead to delusion in later steps, which most students only reach after a few years of serious training if they do things right.

If you go by Bardon's explanation for Spiritual Anatomy (which I use myself), then you are an Immortal Spirit (Consiousness) that is simultaneously incarnate in a Mind (Mental Body), Soul (Astral Body), and a (Physical) Body. These three bodies are vehicles that allow your consciousness to interact on the different planes of existence, but when you die all three of those bodies die and you become Spirit again.

Your Soul is what changes shape and changes back while projecting the Soul (and the Mind is while projecting the Mind), and is the vehicle of your Spirit.
If you spend so much time in other shapes and other worlds that you forget what it's like to be human, then you have a few much bigger problems than what part of you is supposed to remember that. Those fairy tales about people getting lost in Faerie have a grain of truth to them, you know.
You've been a human for many years, and the part of you that most people call the "subconscious" has an blueprint of your existence that will come through instinctually, while your Spirit also has its own blueprint of your highest potential that will come through intuitively. Like a rubber band, no matter what shape you twist yourself into, unless you break something you'll always instinctually and intuitively get pulled back towards the version of yourself that is most natural and harmonious to everything else.
